Financial and Insurance Information

Payment is due on the date of service. We accept cash, checks, and credit cards (American Express, Discover, MasterCard, and Visa). We also offer no-interest payment plans through CareCredit.You can apply online at www.carecredit.com or in our office to see if you qualify.

If you have insurance, please bring your medical and dental insurance to your first appointment and we will file to your insurance company. We do not accept Medicare or Medicaid. You will receive monthly statements. Any balance remaining after your insurance pays will be your responsibility. If you have questions regarding your statement, please contact our office.
